Very thorough sheet. Here are a couple of items I didn't see on the sheet that you might want to consider:
Wind level (a very windy day would obviously effect your lap times and it's good to note that b/c it might not be obvious when you review the video at a later date)
Mileage in/out
engine hours of operation

You might want to add your cold air pressures from whenever you get to the track
Then right before your first session (cold)
then right after your first session (hot)
a space for how much they went up
Then a space for what you adjusted them to (hot)
Then start all over for your next session with your adjusted pressures and see what they've dropped to

you might like to record the driver weight - we saw a consistent difference in set up with SO and WH, where both are typically small but one was actually supersmall.
As well as the roll bar adjustments, check the wing setting.
I assume you don't adjust corner weights and suspension set up (apart from the shocks) on the day.

Here's how I track tire info. In general, I'm usually rushing when checking temps so I need things laid out in a logical manner. Simply put, I set up the sheet so that it corresponds to the order in which I collect data:
Tire Order: LR, LF, RR, RF
Readings: Inside, Middle, Outside
I start with the tires which will lose the most heat and proceed accordingly. Also, I always take temp readings from the inside/out and write it down in the same order to limit confusion.

One key data point)s) are a log of changes that are made session to session ...starting simple: fuel load as an example, to more complex changes: shock settings, bar changes, etc.
And always note the effects. Effects can be noted by lap times, segment times, corner or straight speeds, feel, etc.

I have modified the data sheet to make it more generic as people have asked to use it. Go ahead.
I have three temp conditions for the tires, Hot, AC (after coold down lap), and BC (big cool down lap). I usually have to take the car out of the track and drive around the street to further cool the brakes before pitting.
