Coolant actually in the cylinders? Wow. Yes, a failed AOS could cause that, or the heat exchanger, or a cracked cylinder head. And yet, now it runs fine? Does the coolant level drop when it runs?

As of late yesterday the coolant was not topped off. Will learn more later.

How would this work?
The only way coolant can get in the combustion chambers, and then into the exhaust, would be cracked bores, cracked head or compromised head gasket. And since we all know it's not the head gasket, that leaves the two other possibilities.
On the other hand, there is no proof (so far) that this is coolant. There is a chance water may have entered externally through the exhaust outlets and found its way into any open exhaust valves. Any flood events in the history of this car?

My 2cents
(I'll watch for future postings about this anomaly.)
Whatever the cause (source?) of the coolant, it will be expensive to repair.
At this point, I suspect lingering flood damage, and/or head gasket leakage.
