here is a pic of mine sorry for the glare its very sunny out and i took the photo with my camera phone.
I used the stock 90 degree elbow on the top and shortened it a little, and I used a S shaped hose on the bottom alot of trimming it to shorten it up a little to clear the hood cause its close on my car.

That's right. Your setup should work unless the spring is to weak stay shut until vacuum is applied to the top port.

It should work in both orientations. When you install it backwards (like in the last picture) you will have boostpressure on the valve which may open the valve and bleed of boost. This is not good.
If the spring is strong enough. this should not be an issue.
In theory, when you are under boost, pressure is applied to the top port, keeping it closed.
Special Tools suggests you install it the right way (top picture) just to be sure you are not bleeding off boost prematurely.
