If you remove or have an inoperative vacuum check valve on the line going to the heater core/vacuum canister you will get heat under throttle load, but none at idle or trailing throttle. I know because a buddy broke mine right at the base of the blue part and we spliced the system back together without. I have a replacement in my desk drawer here at the office, but never remember the problem until I'm out driving around

Impossible to get to while it's in the car. You need some mirrors, some crazy 90* circlip pliers, and voodoo magic spaghetti arms to get it out.
mark got mine out pretty easy, but I don't know how he managed. I'll point him towards this thread...

If the clip is in the correct position, it's not difficult to get out. If it was installed wrong as many are, it can be a nightmare.
... oh and yeah, a mirror helps.

20 mins, including refill. First of all my snap ring was oriented with the clips at the bottom of the opening. I used an all to pull one of the ends out of its groove, and i used a flat screwdriver with about a two foot blade to reach in behind the clip where i got it out of the groove and worked the clip out. I was replacing the clip also, so i didnt care that i bent mine up.

I can't believe all these posts and no one asks if the heat is on when the car is accelerating. If the heat gets hot when you are accelerating, the plastic clip under the dash may be broken as the late cars have that problem.
If you want to check vacuum, which porschedoc mentioned, look under the connectors for the crank/reference sensors between the back of the engine and the firewall. There is the heater valve and when you can move the arm through vacuum or if you can get your hand down there, move the arm manually. If the heat problem is solved that way, then its something to do with the vacuum. If not, then its the thermostat or something related to the cooling system.
