Holy ****. Im trying to get my bearings here. Did your dual mass flywheel sheer off...??? Thats crazy. Is the clutch and pressure plate mangled too?
How many miles on that clutch? The first think that comes to mind is the clutch was replaced before but not the flywheel. Or maybe it was replaced and not torqued properly. A quick google shows some other guys have had similar issues:

I have about 20K on the clutch. Still looks new. Yes, before the 60K warranty expired, I had the clutch and PP replaced. However, flywheels are supposed to last up to 100K miles so I never asked for it to be replaced. Besides, dealer would probably charged me an extra $1400+ for the flywheeel.

check out this thread for instructions on how to install the clutch. pretty straightforward if u already know your way around after dropping the tranny.
My writeup is post #85. Look at step 11. Thats probably the most important thing I found to slide the transmission back in.

Thanks for the input.
Tomorrow, I will try to find some 2-3 bolts for temporary use to help me with alignment and to hook/slide the tranny onto the engine.

An additional input: if you have a Power Bleeder to be used for bleeding the clutch, first check if its hose isn’t brittle from age.
Mine started to crack at both ends and now another delay in buttoning her back up! Grrrrr!

The tranny slid like butter without any issues with the last inch or so. Based on various write ups, clutch alignment was critical so I was super cautious here. I even noticed that when the tool is inserted, there is still a small “play” in all directions.
Though I bought the Lisle unit, I still added the inner race collar of the old flywheel bearing. I am not sure if my theory helped but I think after inserting the tool into the new flywheel bearing, it made the tool “aligned AND square” to the new bearing.
