It seems as the washers aren't the same. The top seems OEM, the bottom seems not and is thinner. I also suspect that the failure occurred when tightening the bolt. In all cases whether OEM, Powerflex or other it's a tight fit and you can't squash them as seen on the pic.

Might this be partially a result of overtorquing the fastener? This bolt should only be 63 ft/lbs. The forward bolt is 81 ft/lbs.

I was never comfortable when taking the car to the garage. They simple tighten everything with an air gun.

The steel top hat bushings are critical in that their length prevents this from happening. They bottom against each other setting the bushing preload and clevis width.

Looks as though the top hat bushing length has been altered as you can see the one bushing is squashed.

Please replace the top hat bushings with good unmodified units at the same time the side member is replaced.

hang in there buddy, persevere this, one item at a time and you'll make it out the other end. my bushing install was a nightmare as well. involved having to buy an entirely new control arm. you'll make it through this
