Well not exactly, but if I recall it did involve a stud and a strap.

For future references...

If you are adding some sort of terminal end, washers or clamping device to a bolt...for securing purposes...a longer bolt should have been used...to compensate for the loss of threads needed to secure the additional ground strap or washers thickness.

In fairness I did not add the second ground strap. That was a gift from the PO.

Those are the best!

I think Brian may have diagnosed the failure correctly. That'd point to there being threads further in. Running a stud all the way in may work.
