So here's my question. Do you get the 21K (and .19) from the insurance company (his) or do they just say your car is worth $4,000 and here it is and go away. Isn't there someway to force them to take you back to where you were before the accident? Or do you have to resort to the "my neck hurts because you won't fix my car"

The short answer is no.
The insurance company "totals" the car and then you get to fight about how much it's worth.
The accident was back in 2001 or 02. I think that they originally offered me around $8,500 based on "Auto Trader" type listings. I rejected their offer & they agreed to an independent appraisal. I made the car look as good as possible & supplied the appraiser with as many high dollar listings for similar cars as I could find. The appraisal came back at $11,200 (which was $700 more than I'd paid for the car back in '94) and they paid it.
I bought the car back for ~$1,800 & put it back together. The hit was hard enough that both doors were pinched shut, but there was no suspension damage. After about a year in frame & body shops (with me doing all the disassembly & reassembly work), the car was back on the road.
When all was said & done, I think I had about $7,800 into it (and a "salvage" title). Probably would have made more sense financially to have just gone out & bought another 928, but it was too good a car to throw away.

Slow forward progress
Two things about these Goodridge lines- #1, the banjo bolts that are supplied for the front calipers are too long and need to be ground shorter so they don't bottom in the caliper, and #2), the hardline fittings on the front lines do not have a 'D' shaped flat on them, so they won't fit into the body bracket without either grinding a flat, or rounding out the bracket. But they sure are shiny....
