Reports from people that were there were that he blew straight through 8 without any application of brakes and went through the trap and into the wall. To me that seems like possible brake failure or a medical event.
Not sure how fast vintage F5000 cars are but modern road cars are 240, 250, 260+ kmh coming into 8 at the end of the straight. Absolutely worst possible place on track to have your brakes fail or some other issue that would preclude you from getting on the binders. Tragic event, his wife was there at the track too apparently

I was there on Friday and spent time in the F5000 tent looking at the cars and chatting with very friendly drivers. I had not seen an F5000 car up close in some time and it reminded me of the Can-Am cars that I saw at VARAC two years ago. Light weight, monster motors and virtually no crash protection for drivers. I thought at the time "These things are dangerous". I did watch practice and they are fast and noisy and I suspect require high degrees of skill and bravery to drive at speed. I do recall that an F5000 driver was killed a number of years ago (10??) when bodywork came loose on Andretti and he lost control.
Brian Redman, VARAC Grand Marshall this year just came out with a book that describes how dangerous cars of this vintage were to drive. Obviously, nothing has changed.
It is so sad when an amateur racer who is a successful business person with a family pursuing his/her dream loses their life. Not fair.

The problem is the cars were dangerous back in 1972 and just because its the 21st century doesn't make them any less dangerous. FYI: This series doesn't require a HANS device. (Not that it would have helped) I spent an hour with the crew from 8&9 and some were shaken up but the debriefing went well.

Was nice to see tribute to Jeff Green by entire Indy Light field at RA.
