Also: if youre registration is confirmed they will have sent you an email last week. Scroll down to the bottom of that email and the tech form should be attached

Also, remember that the Carolinas region requires the form be completed by a shop that is on their "Approved Tech Facilities" list. Out of region shops are listed on pages 3-4.
