There are a couple, is this on a 95 or a 96 +
On a 95 the main vacuum line is right above the engine fan shroud on the intake plenum. One line goes behind the plenum, & up next to the throttle body on the passenger side, there is a actuator.
The other part of the line goes to a 4 way then to the heater fan servo (for reverse), a vacuum canister, & then the other flap under the intake plenum. lastly, to the heater valve actuator in the front of the car, on the passenger side in the top of the trunk under the black plastic cover.
The last vac line from the intake goes to the fuel pressure regulator.
Their might be more, but this is just off the top of my head.

How can I check to see if the hose at the actuator has fallen off...is it possible to see the actuator?...or is it so buried behind the engine seeing it isn't possible?

You can also hook up a simple vacuum tester to the lines & see if they hold vacuum.

Burning Brakes
If your car has been driven recently have someone turn on the key while you are by the engine. If there is vacuum stored you will hear a noticable click in the center of the intake, this is the actuator moving the flap.
