Default Clutch failure

So here are pics of my exploded clutch. Clearly it failed. Question is why. Any input is appreciated. 95 993 lightweight flywheel. Clutch has lots of material left. Two rivots popped.

How did you find out it failed? Noise, slippage, notchy shifting?

I dropped the engine and took the trans mission to have it gone through. Only real problem was getting into second. I don't think the clutch was the entire problem as my syncros were bad. Anyway, I took the clutch apart just to see if the rear main was okay while I had it out of the car. Seal was good but clutch, not so much. I am going to go pick up the box from Mr. Copan tomorrow but will need to order a new clutch before I can start putting it back together.

John,

Take note of how off-center the splines are in the center of the disc. I think she may have been subjected to some rough use ,.....

I noticed that and the place around the edge that was ground down. I think the rivots went and then the center went askew? The second and third pictures show the defect. I am going to show it to Brian Copan today when I pick up my gearbox to see what he thinks. It's hard to diagnose from pictures sometimes.
