Default Voltmeter pegged

Recent development - the voltmeter is pegged while the engine is running. What might cause this? The voltage at the battery is 13.5V.

Sounds like a faulty gauge. If you have confirmed proper charging at the battery, the gauge is suspect.
Could also be a grounding issue.

Are there common ground points I should check and clean?

Either way, I'm going to be removing the gauge cluster at some point to redo the dash lighting reflectors. I can address the voltmeter at that time if it turns out to be bad.
