Default Is this an H4 or H5 headlight lens?

One of them has a stone chip in it and I need to replace the lens but not sure which one to order.

H5, it has the alignment nubs on the face.

If you are replacing the headlight, you may want to consider upgrading to H4's.
You can buy the whole kit from vertexauto or other places, the H4 adapter plugs from AJUSA and have the primered H4 trim rings sent to Eric at bumperplugs.com for paint match.

I did this on my RSA and was very happy with the results, much better lighting, same wattage/no relays, wiring mods required.

One advantage of upgrading to H4's, however, is the ability to use larger watt bulbs - that does require the installation of relays and upgraded wire, however.

To the untrained eye, what is the difference between the H4 and H5 headlights? Are the H4's brighter? Different cosmetic appearance? Just trying to weigh the benefit since I can buy a single H5 lens for $135 and the H4 conversion will be in the neighborhood of $639 including painting the trim rings. Is the improvement worth that much difference in money? (I checked with bumperplugs.com and it will cost $200 to paint the trim rings assuming they are already prepped and Vertex sells the H4 kit for $439).

Huge improvement in quality of light - both pattern and brightness.

My local autobody shopped painted the rings for CDN$75 at the same time they did some stone chips for me.

Has anyone successfully replaced just the lens on an H5? They're not made to come apart, as far as I could figure out. PET shows the lens and reflector as a unit, and that's how I was quoted - about $400.

I'd still recommend the H4s. They're better. They look better and you'll see better (even with stock lamps).

I replaced my H5 lens with H4 lens. It's tedious to chip the lens off the H5 lens to the point where it's glued onto the reflector without affecting the reflector itself. One lens was already broken thanks to a brake drum weight which flew off a truck in front of me and went through not only the lens but the reflector itself and ended up in the light bucket. A little epoxy sculpturing and chrome paint repaired the reflector. I then epoxied the lens to the reflector making sure it was perfectly sealed from moisture. Silicone worked better on the other lens in that it will be easier to take off the next time. There's less adjustment because the H5 rings fit tighter to the lens. I've since replaced with a new set of H5s and it couldn't have been more than a week and a stone cracked the lens. I suggest those plastic films you can buy which helps from stone chips but I don't think they'd stop a drum weight at highway speeds!
