Default Over boosting?

I was driving my car a little harder than usual today, when I noticed my wideband going nutts around 4.5k rpm's, it would show my a/f around 20.1. I did a few runs thinking maybe my wideband needed calibration and thats when i noticed my boost gauge pegging at 19psi. I have my reliaboost setup at 16psi and I have a shimmed wastegate. I have allready checked the line between the reliaboost and the wastegate and it looks ok. Your help would be greatly appreciated.

Take the Reliaboost out, open it and inspect it.
Also, clear the "spike" hole with a pin or needle.

If the line from the mbc to the wastegate really is in tact, then Id lean towards the wastegate itself.

Thanks Cyrus, i got the reliaboost working but now iam having problems with the Lm-1 under hard acceleration after a few seconds from reading 11:7 to 12:3 it jumps to 20:9%02 which is the reading for regular air. I have checked the program on the LR digital maf (power perfect unit) and the map is ok, the only 2 things I still need to check are a bad WB oxygen sensor and maybe a Air leak, but all the symptoms lead to a bad oxygen sensor I think.
