Well, after a little research, it looks like Volara and Ensolite are the recommended CCFs. Volara is cheaper. Ensolite is the stuff the RAAMaudio recommends, but it is more expensive. From what I've quickly read, it sounds like these foams don't actually offer much in the way of acoustic blocking or adsorption, but they are good "decouplers" between isolation layers of other materials.
1/2" Volara is pretty easy to find online. 1/2" Ensolite is harder to find. I think that I am going to go with 1/4" foam from this source and use their mass loaded vinyl on top of it:
I'm still debating using the 100% or 25% coverage methods for the vibration dampener material. It seems most here have gone with 100% which certainly looks more impressive.
