Race Car
It's ironic this thread came up, I have a drive on lift and never bolted it to the floor. Everyone said that it should bolted be but the installer did not. We were working on my wife's big caddy and while yanking on things the lift started to sway. Well my brother purchased all the bolts, drills Etc for Christmas and we just finished tonight locking everything down. I guess he wants me around to keep his 86.5 running for a while, so anyone else who has a lift I would highly suggest the extra level of security.

Burning Brakes
I also know the feeling of a car coming towards you, caused by failed scissor jacks.
Ever since I ALWAYS put one or more spare wheels under the car, when I am working on the car.
I also once had a hydraulic jack which failed, it is unbelievable how fast the car dropped down. No way anyone would have survived in case he or she would be under the car.

i had a similar experience with a 1983 jeep cherokee i had in college.
i was always working on something with that thing, as i used to 4 wheel a lot in it..
I was changing out the transmission and rear end and i had the rear of the truck up on two hydraulic jacks and had just come out from under to go pee and as i was wiping my hands it started to move then BOOM crashed to the ground....
i also worked at a marina in high school and was under a 40' sport fisherman painting the bottom. My dad came over to go out on our boat and freaked about me being under that boat, and the way it was on the permanent work rack... he made me get out from under the boat and went in to talk with the owner ( his good friend) about the situation. while he was in side and he and the owner were looking at the boat from the owners office window.... BOOM the boat fell off...
ever since these two close calls i am uber careful being under anything....
