Default Bad ISV? Vacuum issue? Something else...

I was on the freeway the other day and after pulling off, my car suddenly started running really rough. Lacks power. At idle I feel like I'm in some old Chevy with a 454 or something. The sound is bad too. Like it has an exhaust leak. I've never had any trouble with the engine, and it has always run strong despite high miles. At higher RPMs it is not nearly as noticeable. Quite frankly it feels like I'm missing a cylinder.

Thoughts?

Don't know if this helps, but it is worse when cold.

Thank you, I'll check it out. I'm going to throw some new plugs and wires on, also. Wires aren't in the greatest shape.
