I've never done it myself, but basically it replaces the circular "gear" that the throttle cable attaches to. I believe it's smaller, and makes the throttle plate open quicker than it otherwise would. You're not going to see ANY improvement in horsepower from this modification, but based on what many people on here have said, it makes the engine a bit more responsive which apparently is sorely needed.
I personally have not done this yet, but I might when I get to that point on rebuilding the car.

I started out by following the instructions that this guy gives,LINK
But i soon found out there was no way I could make my cable fit through the hole that he says to drill; so I just cut the entire ear off of the cam and ground it smooth.
I've never owned an aftermarket throttle cam, so I can't really compare the DIY one to the real deal.

basically what the cam does is changes the relationship between the gas pedal and throttle.
stock:
the first 2/3 of pedal moves the first 1/3 of the throttle plate, due to the little arm
the last 1/3 of pedal moves the last 2/3 of the throttle plate, thats the rounded part.
aftermarket cam:
linear relationship, 1/3 moves 1/3 etc.
what this does is makes the pedal very quick and responsive at first, but about halfway down to all the way to the floor the car does not accelerate much more, so it takes a little getting used to.
I like it, makes it easier to drive....

Honestly, if the aftermarket one feels like the one I made, I wouldn't spend my money on it. It feels basically the same as just mashing your foot down fast and hard with the stock one. I did noticed the most difference around 3-4k RPM in second gear; car felt alot snappier here, but not much different anywhere else.
Overall, I am pleased with it as I didn't expect a huge difference. I would recomend picking up a spare stock one to modify just to see if you like it enough to drop $40+ on one.
