They have plenty of competition in the US, but I don't see anyone fighting for your hard earned buck in the medical field. If that were the case, it would not cost $10,000 a day in a US hospital.
Competition for your buck when buying a HD TV works well...not so well when you are just about dead and they know you need help...no choice in the matter.
BTW, my hospital stay all inclusive including the operation was €483 per day. That works out to about US$625 per day and I wasn't even a resident of the country. Service was first rate! So how can it cost what it does in the US? I know, do in all the lawyers.
