Has anyone been able to come up with a reproducible explanation for why wireless Carplay sometimes connects immediately and why it sometimes refuses to connect? I know I'm not the only one...

Unfortunately it seems to be the nature of the beast with the PCM handshaking with the phone. There is a lot happening behind the scenes when the Porsche PCM boots up I suspect. If something delays or throws off the timing of an internal application, it may exceed the time out value for connecting to the phone.
I've found that a reboot of the phone once is a while seems to help with the stability of the wireless connection.

Two ways, a soft reboot is pushing the left **** in and holding until the screen goes blank (about 5-7 secs maybe). You won't lose any of your favorite stations or paired devices.
Hard reboot, disconnect the battery for a bit. This is a last resort option if the PCM ever crashes on your hard and you don't want to head to the dealer. I believe this will lose all your information and require re-pairing of your devices.

Next time it refuses to connect, check your phone to see what it is connected to via wifi and BT.
