My WAGs:
Manual? Shifter linkage. Internally a shifter fork failure.
Auto/Tip? Shifter linkage, problems in the tranmission valving, low fluid.
In the case of a manual a look/see from under the car while someone moves the gear lever around might make it clear the linkage is at fault, or not.

Easiest thing to check and very common is the shifter cables popping off the clip at the back of the trans. Easy to spot lying on the ground looking up at the passenger side of the back of the trans. The cables clip to the body of the trans and if they come off you are missing the ability to engage half of the gears.

paul996's comment. I had the 2-4-6-only phenomenon and it was that issue. (except, of course, the other cable that is pulled by the lever moving forward / back)
