It’s weird. The mods are pretty common for these cars. I wonder what really cause the failure. I never seeing such a failure in these cars.

'other turbos' could mean a lot of things, the stock VTGs will only push so much air.. a non-stock turbo with more boost, with a not-ideal engine tune without the low to mid range torque turned down, combined with a supply of bad gas can lead to this.

Maybe take those turbos OFF, and determine what they are. Kudos to rebuilding that yourself. Clearly, you know what you are doing.

The turbo's are modified by 9FF a German tuner in Dortmund
They now what they are doing
