In formal writing, Ph.D.'s aren't even addressed as "Doctor Smith". It is merely "Dr. Smith".
In Germany, the full professors are addressed as "Herr Doctor Professor Johan Schmidt", which is often abbreviated as "HDP Schmidt". If you want to irritate one of them, just address him as "Hey Johan! How ya doing!" on the phone. Actually I don't like to irrirate people like that- if they want a title then why not give it to them. But I did *accidentally* first-name an HDP once and I got the German moment of disapproving silence followed by a stern "...Ja".
