Default insulation (deteriorated/dry-rotting) insulation coming out of heat/AC vents?

Hi
A little odd to see this black deteriorated insulation coming out of the vents.... it's deteriorated to the point of crumbling instantly in your hand....

Any thoughts on where this is from, what caused it, and how to remedy?

Normal wear and tear on insulation? But how did it get inside the vents?
An indication of another problem?

Thanks

Mine has it as well. Pretty common problem, as I recall. I've been ignoring thus far, but will have to fix at some point.
