So far Andrew has a good start on what may contribute to failures, but its not been proven that any one of those things accounts for the majority of failures.

The reason the pipes failed is plain obvious. You even known the answer, but since acknowledge it would contradict everything you have ever posted on the subject going back more than a decade you continue to double down in this "mystery" in a juvenile attempt to save face. You are deceiving no one.

In this thread, the reason why the fittings come out was intimated to be because of "bad glue." Andrew posted a more properly broad list of considerations regarding why the pipes fail. I agree with him on his extended comments.
