The brake wear indicator goes on when there's an open circuit in the break wear indicator wire. Normally this should occur when the wire wears through due to thin pads but the wire/circuit could be "open" for other reasons--bad connection, damaged wear indicator, etc. Or it could've worn through prematurely if not properly reinstalled at your last brake pad change.

If I faced this problem, I'd visually check that all of the indicator wires were in good condition and that they were all plugged in fully. If I found nothing out of order, I'd check resistance of each using an ohmmeter--high resistance (open circuit) would indicate a bad one. Fortunately they're not real expensive parts ($25 or so IIRC) and easy to install yourself.

The sensors could also un-clipped themselves from the pad itself if the clips were broken or the sensors re-used instead of replaced. And then once they are flailing around - perhaps it is possible it broke lose or became damaged.
