Question Perplexing SD Card issue (991.2)

I recently purchased a 128GB SD card and have filled it with approx 2200 songs / 16GB. After letting the PCM finish indexing and accessing album covers via Gracenote, I notice much of my music is missing.

I tried a small (128MB) card loaded with only with a couple of the "missing" songs and the PCM sees them without issue. This tells me it's not the files themselves, but how they're stored on the card.

The Owner's manual states the PCM supports "SD cards up to 128GB". Given there are two SD slots, could they actually mean "64GB + 64GB". ie two SD cards for a total of 128GB?

My 991.2 has a single 128GB card containing 823 songs encoded into flac files occupying 16.1 GB of storage. The music is organized into 93 folders with 29 folders at the topmost level. I cannot remember the details now but I think that the folder/directory hierarchy is somehow restricted. I have not noticed any music to be missing.
