South Dakota: 92 most places but a few carry 93. You have to be careful though because sometimes the 93 has 10% ethanol. Shell stations around here generally carry the good 93.

Yes Canadian octane ratings are the same as US. Alberta has 94 octane which is a 10% ethanol blend in some stations, but most stations are 91 octane straight gasoline.
Louisiana-93 on every corner, 94 at a few stations. In New Orleans pre-katrina we had a station with 100+ octane. So there may be such gas in some other part of louiana.
