Your symptoms were just SO similar to what happened on my car last summer. On mine the set screw disappeared completely and the rotor was spinning freely.

I was very lucky because I was leaving a drive-in burger joint only about 1 mile from the Lindsey Racing shop when it happened. I was able to idle my way over to their shop and they helped me find what went haywire. It only took us about 5 minutes to find the loose rotor.

After that incident and loosing a CV joint also while leaving that drive-in I've quit going there
