Okay, so.. I've got vitess AFM chips. I passed emissions last year with them, so I don't know what the deal is. Anyways, I failed this year with the following for a 25/25 test:
HC ppm: 158
CO %: 4.85 <-- FAIL
NOxppm 189
CO+CO2%: 16.7
Okay, so I do some searching on here, and some people say that having a bad O2 sensor can cause you to fail on CO%. So, I get the Bosch universal sensor, splice it on to the old plug, and go get retested today.
Guess what happened......
HC ppm: 171 <--- FAIL
CO%: 5.99 <--- FAIL WORSE
NOxppm: 254
CO+CO2%: 17
Okay, so now I'm really fru$trated. I get home, swap in the stock DME chip, go get retested. I think the guy was tired of seeing me.
AND VOILA! She passes with flying colors!!
HC ppm: 47
CO%: 0.14
NOxppm: 319
CO+CO2%: 14.5
Moral of the story: Keep your stock chips, you might need them!
Cheers

I should note that I had the fqs on 2, which, for the chips I have, is the "slightly richer" setting for 15-16psi with an aftermarket WG. I had been intending to change it to 1 for a while... I just didn't want to take the chance with the test and have to do it a fourth time.

I pulled the VR II, installed the APE II, and a bottle of Guaranteed To Pass from Kragen on an 8th tank, and had better numbers than the last smog.

If the FQS was set to "slightly rich" of course the CO will be high. Why didn't you run with the FQS set for std AFR? Why did it pass last year and not this year, the chip didn't change, did you change the FQS?
In any event, you got it to pass.

I have the VR K26/8 AFM chip and passed with flying colors, even better than the OE chips I used the first time I smooged the car 3 years ago. I would suspect something else was out of whack than just swapping the OE chip.
