Default Wheel spacers and stuff

So I'm having wheels fabricated to 11" wide. What size spacer do I need to go from ET52 to ET45? Who makes good quality spacers?

... But if you are having the wheels fabricated, why not just get the correct offset on the wheels, and no spacers needed?
