Yes, every 2 years.

Some counties are considered rural and are exempt.

However; if you register your car as "HISTORIC", you're exempt.
My '87 is registered historic, with personalized tags.

Historic - car must be at least 20 years old and the model is no longer in production.

There may be some other restrictions.

The emission inspection is NOT like California. They don't lift the hood or look under the car.
My last emission test was 'tailpipe' ONLY.

If your car is running properly, getting it through emissions is not a big deal in MD. My '85 cruised right through in '07.

Here are the state standards:
Hydrocarbons (HC): 1.8000 GPM
Carbon Monoxide (CO): 30.0000 GPM
Oxides of Nitrogen (NOx): 2.8000 GPM

These are my results:
HC: 0.0569 GPM
CO: 0.4170 GPM
NOx: 0.1031 GPM

The way I see it, having an emissions check is a cheap method of making sure my car is running properly.

If you are in MD and have the air dam in the front like the S4, they physically won't be able to test your emissions. You can go to the office and say that the car is too low in the front, and they will walk out to confirm. Then they write you an exemption for two years. I think next year they are putting in flat lanes instead of drive-up ramps, so that exemption will disappear, but it's not very difficult to pass, and it is good for 2 years, so if you have it done now, you can at least get two years of exemption before you have to worry about it.

Also +1 on the historic. The only problem with historic is that cops (if they feel like dicks) can pull you over if you aren't driving on a holiday or parade or whatever. The law says you can use it for "occasional driving" that isn't event-related, but my dad was driving his '72 corvette late one night and a cop pulled him over and harassed him about the plates. It didn't matter, because my dad wasn't even speeding, and basically politely told the cop that he could eff right off, but if you want to use it as a DD, don't drive by the same speed trap every day or they might notice, because it is technically illegal
