1. "According to my print out, HC and CO fail, NOx passed. relative air/fuel is around 30% rich." - mjensen -

2. "I realized that the afm to intake o-ring needs to be replaced. The afm was extremely loose" - mjensen -

Since #1 indicated a rich running mode, the #2 would cause the opposite effect and
thus not the problem of the emission failure.

Check these:

1. fuel pressure
2. misadjusted AFM (if used) or a bad MAF (if used)
3. a bad water temperature sensor, connections, or grounds
4. bad O2 sensor (if used) or grounded lead
5. check that WOT input is not grounded
