How critical is this when replacing the clutch?

If the surface is not worn then I don't bother on my own cars, I just scuff up with sandpaper.

When looking for flywheel defects,blue spots if very long and obvious roughness of the engagement surface would dictate turning or grinding of the contact surface

If it's out of balance with your pressure plate or clutch having it balanced while your in there is a good idea. That does involve surface machining.

It's important to note than when you have a flywheel resurfaced, they also remove material off of the shoulder where the pressure plate sits. A competent shop will have all of the spec dimensions for every model - you want the step dimension correct so the pressure plate applies the right amount of force on the clutch disk (and the engagement point on the pedal throw will be correct).
