Does anyone know where I can get this exhaust flange?

Try your local muffler shop. I have bought a variety of bungs, hangers and other pieces from a nearby Midas Muffler.

The part you are looking for is called a "header reducer". Summit Racing has them. Some reducers are two pieces and may require welding.
