Ok, I've got Photoshop CS, give me a hint as to how your doing this.
Master the different "selection" tools along with the "rubber stamp" tool, and (in PS7 and CS only) the "clone" tool, and you're there! Budget about, oh, the next 3 months of vacation time to get great at it (so you REALLY can't tell is been 'shopped). ymmv.
