From my 993, guessing it will fit many others.

Small ballast DDM (I think this is the latest version here: http://www.ddmtuning.com/Products/DDMPlusHIDKit). I think it retails for about $75 new.
The external part with the resistor got slightly warped as I had them too close to the low beams in the housing. But they both work great. I only took out as I switched the whole system to something that included low beams also.
I'm guessing shipping will be about $7-8 anywhere in continental US as it will fit in the small flat rate USPS box nicely.
