Custom Earphones
So, who makes good custom earphones? My current set became uncomfortable with my last helmet change. Figured I'd get some new molds made and this time, if possible, put the helmet on while they set. Then send those out for new headphones.

^ Sound like they are made by the same company that made my current set, Precision Labs. I've not had any problems with these, but thought I'd explore the manufacture options available before ordering a new set.

I recommend using an audiologist to do the fitting.

A local audiologist can certainly create the mold. I've had them do it a number of times for musician earplugs.

Several companies build the ear buds. The key part is the mold. Starting with an audiologist familiar with taking these molds is your best bet.
Once you have a good mold, a company like Speedcom or Racing Electronics (REI) will store the mold and you can order a replacement set from that stored mold without getting a new mold every time. You just call and order a new set. Comes in a pouch cleverly marked with the date of manufacture.
My car is loud. I don't drive without them. I use them for when alone, racing or instructing. Much better than ear plugs.
Hint: I like a longer cord than the default build. All I had to do was ask them to make it longer when I ordered my next set and they were happy to do so.

JH Audio makes very high end custom ear pieces, first quality and priced that way.
