Solved.

Ended up being a very stubborn air pocket that didn't show itself until a longer drive ~90 minutes. The temp gauge would flash up to 190 and settle back at 180 within a minute or two.

I went the old fashioned way and burped the system using a large funnel. I ordered the Lisle VW radiator filling funnel off amazon and it took 3-4 cycles with a lot of hose squeezing but it worked!

Yup, thanks to the guys here for sticking with me through this one! I've never had a cooling system this hard to fill.

Dunno if you tried it - and with a Cayenne it's probably not all that practical. Trick used on some cars with the filler up front is to drive the car up on some ramps so the filler is the high point in the system. Then squeezing hoses and revving the engine and blasting the heater usually gets it done..
