Default Dual mass flywheels repeated failures. In spite of little wear on my clutch

Twice now I've dropped the trans on my 01 S to do the IMS bearing only to find the DMF has failed. Initially @ 63K and now 40k later. There was minimal wear on the clutch discs. I've owned it since new so know it's not been beat on. Both DMFs were LUKs. Does anyone have insight as to a solution? I need to put this back together for the upcoming DE season but don't want to repeat the same scenario for fear resembling the definition of "insane".
