Misfire ....blown out spark plug 911

Has anybody ever come across a spark plug been blown out the cylinder head ?

Yes, but not my Porsche cars.

Ford had this problem in their 4.6L engines. Had a plug shoot out of my Expedition. Shop had to tap and place a steel insert.

What did they put it down to ?
