Is this is the same as internal alignment that they test? Where they do a full roto on the crankshaft, check and repeat? Trying to figure out if I'm conflating "internal alignment" and "camshaft deviation" or if they are the same thing vis a vis IMS prequal inspections.

No, the camshaft deviation is a value you can find using either a Durametric or Piwis diagnostics unit. The max cam shaft deviation threshold is 6 degrees.
