I'd say his wheel loosened a bit when he slammed into that 6 inch high center median. I also agree with others this was intentional as he slowed significantly definitely in control in the far right lane then floored it again. If not for the video being released his insurance claim would have stated he swerved to avoid a Great Blue Heron.

I see smoke coming from under the rear wheels at all times. As a matter of fact, in the last seconds of the video when the car is slowly driving towards its resting place at the utility pole, I still see the rear wheels moving faster than the front. Which kind of supports the idea that the throttle was stuck.
Now, whether it was suck by itself or pushed by a panicked foot is a separate questions. But how else would you explain this?
