I'll close this thread out..

I don't know what was wrong with the car. hah

After chasing the same situation above for over two years (could get it started wouldn't stay running for long) I dumped the stock harness/Maf/Coils/ECU and went with a Link ECU stand alone and LS2 truck coils.

Now the car starts with the first crank cold, hot, and in between. I wish I had done this sooner instead of chasing my tail for 4 years.

My un-educated guess is that the add-on wiring that I did to make the S2 ICM fit in my 8v chassis wasn't up to snuff. So those would fail with regularity (if they worked at all out of the box). But this is just a guess, since I also replaced the AFM (tested OK), stock coil (new), and new injectors (450cc high impedance vs 350cc stock low impedance (also new)), and switched from the stock speed sensor to a 36-2 trigger wheel.
