Does the 718 GTS sound the same as a 718 S with Sports Exhaust, or did they do other stuff to the exhaust and associated systems for the GTS model?

The short answer is no. I believe they changed the design of the headers on the GTS and perhaps other minor modifications. I guess the sound was different enough for Porsche to decide to skip the soundaktor with the GTS...
I have a GTS but have only test driven an S without PSE so I don't have much real world feedback re the difference in sound.

I test drove a base, then an S with PSE and was almost sold on the S and upon returning to the dealer from the test drive there was a GTS parked a few cars down from where we were parking. I asked about it and the salesman offered a test drive in the GTS. I was sold immediately, yes, there is a difference.

GTS also has those "pop & bang" gunshot noise when u lift throttle at redline. S doesnt...
I test drove them back 2 back with my PSE C2S at pebble beach... and that 718 GTS is louder

I've driven both at a race track back to back and also listened to both from the outside during startup and revving. There is a big difference - the GTS sounded much better. The sound was a bit deeper and there's more pops and crackles when you lift from the throttle. From the inside, it also sounds better because the GTS has the sound symposer which funnels the intake sound into the cabin.

Bought a GTS without driving one (couldn't find one locally). I loved the S but when I got the GTS you should've seen my smile when I started her up for the first time. The GTS is much meaner sounding.

Look at the spec sheet, performance, look etc - it comes together pretty well. Porsche are great at adding features and extracting "value". People buy what they like, what moves them, what they enjoy - if are prepared to pay more to get what they like there's little to justify to be honest.

P.S. I get the allure of options; my other car is a loaded ‘18 Macan, so with that I get to experience Porsche’s “luxury” features. For me it’s a nice balance; pure 718 and tainted Macan. 😎

Fair enough, I'll happily disagree - I'd take the 718GTS over the base 991.2. I did the same when the 981 GTS came out I also own a 991 II GTS - the 911 isn't the end game and there are plenty of 911 owners that own bath Caymans and 911s. Many appreciate the strength and weaknesses of the different driving characteristics - they are different, one is not better or worse than the other.

Surprised you went for a Macan and a base Cayman after the 997 GT3, the series II of that car is one of my all time favourites.
In my view its easy to justify the 718 GTS - in terms of price for performance its probably one of the best Porsche sports cars going around.

Its certainly a lot quicker, but then again so is the 718 GTS. The 718 is better balanced and just as quick as the base. Also far easier to drive unless the 991 has RAS, which the base doesn't.

And I would have loved a 718 GTS, but I maxed out my current budget with the Macan (had to make the wife happy). Also had to resolve a ton of negative equity. If I told you the cars I’ve been through in the past few years you would be dumbfounded... 🤓

I really like the 991.2 GTS, in some ways it's the ultimate compromise - just about as quick as GT3 but a bit more real world liveable. You can soften them or harden to taste from the options menu. Yet equally it can be quite a sleeper in debadged form.
A bit of a stealth weapon - or wolf in sheeps clothing 😀. Having said that they're all good in the current line up.
Likewise the 718, it's a really nice drivers car very high engagement factor - far too much was made if the engine changes. The reality is the chassis and handling now have the extra oomph that should have been there a few years ago.
