Ok, I worked out the unknown item.
It's Microlon oil additive. http://www.microlon.com/automotive_engine_treatment.php
Very popular to use in Japan and a bit expensive depending on which one you use.

so final translation:
top left - Oil change 10th March 2000 61000kms
middle left - Microlon Oil Additive 10th May 2000 62400kms
middle left - Battery change 17th July 2000 63142kms
bottom left - Oil change and Filter change 1st October 2000 63400kms
top right - Oil change 15th February 2002 67439kms
