It seems hit or miss but half the time when I stop for gas the filler door is locked and I have to use the manual release to unlock the door. Does this actuator fail often? Btw, the doors are unlocked when I stop at the pump.

If your doors are locked so does the fuel door so maybe the alarm armed or doors locked while you paid for gas also check the small rubber grommet attached to the car makes sure the fuel door actuator lines up with the black grommet hole sometimes it gets crooked and doesn't align properly when locked.
