The former. I can upload a screenshot but the painted trim is indeed on the doors and it is full leather interior... what I like is the middle of the door panel is Alcantara as well to go with the arm rest and door pull/handle. That part is just black plastic if you don't have the full leather interior as I had in my build originally.

Also to clarify on the GTS the painted trim is a $0 option...

I'm not surprised. Sapphire is a rich, deep color and terrific for the GTS. It is a bit bold but certainly not over the top. I imagine it will go very well with the black highlights/details showcased by the GTS exterior.

you are a bad influence ...

I had the interior trim painted on my '07 997.1 GT3, it was cobalt blue ... I have to say that for me it looked great, and is *very* unique...

Yeah for me it is both unique and helps add some brightness/color to a relatively dark interior... I love the Sapphire Blue color and bringing a bit inside seems like a good idea to me.

You are so right. The 991 certainly pulled me in. The combination of the upgraded interior, improved performance, PDK improvement, and the pure beauty of the car made it a must have for me.

Like Vise says, the painted interior does add color to an otherwise dark interior. One of the things that I've thought about (if I go with the GTS alcantara pack) is to add either the vent slats or the vent surrounds painted. It's a much smaller and more subtle amount of exterior color, but it may add just enough contrast to make a difference.
